Lake County Jail Ministry Release of liability
I, __________________________________________, have been fully advised of the nature, type and scope of the Lake County Jail Ministry (LCJM) Community Life Program, and I choose to voluntarily participate in the program.
In consideration for being permitted to participate in LCJM’s Community Life Program, I, on my behalf and on behalf of my heirs, assigns and successors, hereby irrevocably and unconditionally release, acquit, remise and forever discharge LCJM and all of LCJM’s volunteers, officers, directors, agents, principals, affiliates, servants, employees, successors, assigns, heirs, executors, administrators, officers, directors, members and any other persons, firms or corporations charged or chargeable with responsibility or any liability or responsibility whatsoever, or any other entity in privity with them (the “Released Parties”), from any and all claims, demands, actions and causes of action of any kind or description whatsoever, whether arising out of contract, tort or otherwise, in law or in equity, which I had, now have or may have against the Released Parties, whether or not now known, including but not limited to any claims for property damage or personal injury arising out of or related to my participation in the Community Life Program. I agree and covenant forever to refrain from instituting, prosecuting, maintaining or recovering upon any right, claim, demand, damage or cause of action that is within the scope of the foregoing release.
I have read this Release in its entirety, fully understand its terms, and understand that I am giving up substantial rights – including my right to sue. I expressly agree and assert that participation in the LCJM Community Life Program is voluntary, and I knowingly assume all such risks and elect to proceed with the participation despite all the risks. I represent that no promise, inducement or agreement not expressed in this document has been made to them. I acknowledge that I am signing this Agreement freely and voluntarily and intend, by my signature, the complete and unconditional release of all liability to the greatest extent allowed by law.
